PHARMACY STRATEGY: Medication Management Clinic (MMC)

Expanding ambulatory services is an important strategy for health systems. One option to do this is the creation of a pharmacy department-run Medication Management Clinic (MMC).

MMC integrates clinical pharmacists in a referral-based model working under a collaborative practice agreement (CPA).

This model has proven to be effective in expanding support for more patients with the 340B program and increasing clinical continuity.

Collaborative Workflow:

1. Provider / RPH Identify Patient
2. Refer to MMC RPH
3. Visit with RPH 
4. Assess, order, and educate under CPA 
5. Financial Barriers Removed
6. Prescription Sent to Pharmacy Documentation sent tp Provider 
7. Follow-up visits with RPH and Provider 

Service Benefits

• Create clinician capacity and improve patient access
• Collaboratively support clinical outcomes for patients
• Improve financial margins 

Some organizations have called such clinics chronic care or chronic conditions, but I prefer Medication Management to align with our national initiatives around standardizing to this term.

9. Costs too much - The investment in your future will be worth it.  You will increase your options for raises, bonuses, and promotions.  In addition, there are many lower cost options online these days. 
10. I already have one - Congratulations!  Consider updating it every 6 months
